Superbill audit: is yours claim-ready?

Open your current superbill template and tick every field it already has. You get a score and the exact gaps to fix — before a client's out-of-network claim comes back.

Your superbill score

High rejection risk

A practical checklist, not billing, coding or legal advice. Requirements vary by plan; reimbursement is never guaranteed. Confirm current CPT and payer rules.

Questions

What must a dietitian superbill include?

Provider name, credential, NPI and Tax ID; the client's name, date of birth and address; each date of service; CPT codes (97802/97803/97804) with units; linked ICD-10 codes; place-of-service code; fees and payments; and your signature. Some plans also need a referring provider.

Which place-of-service code do I use for telehealth?

Commonly 10 when the client is at home and 02 when they are elsewhere, but plans differ — confirm with the client's plan.

Does a complete superbill guarantee reimbursement?

No. It only gives the client what they need to submit a claim. Coverage depends on the client's plan, diagnosis and payer rules.
